1. Download the Transfer/Reactivation Form from the National Website or fill out the one attached below.
2. Make your check payable to Phi Beta Sigma Fraternity, Inc. or complete the Credit Card Authorization attached below. A list of fees can be found below.
3. Mail the completed information to:
